During a home visit, a Case Manager, who specializes in resolving
problems of seniors, will assess needs and provide options in which
to meet those needs. The Case Manager will make appropriate referrals
and will follow-up to ensure that the senior receives appropriate
services. For example, a Case Manager may act as an advocate in
resolving conflicts, or assist the senior in obtaining services such
as home delivered meals or a chore worker.
